Resident Evil Requiem almost repeated Ethan's ordeal

Resident Evil Requiem notably almost presented its new protagonist, Grace, with one of Ethan Winters' wildest experiences from Resident Evil 7: Biohazard. In a recent interview, the game's director, Koshi Nakanishi, and producer Masato Kumuzama shared that there had been discussions on potentially putting Grace in a position where she would suffer a similar traumatic event to losing a hand, like what happened to Ethan. In early sketches for the story, the idea revolved around Grace losing a leg or sustaining a bad injury, which developers believed would add to the horror. The idea eventually fell out of consideration before reaching the final product.

A Reference To Some Of The Historically Shocking Resident Evil Injuries
The idea stemmed from some of the shocking injuries committed against Ethan in Resident Evil 7 and Resident Evil Village. In Resident Evil 7, Ethan has his hand cut-off by members of the Baker family but then sticks it back on his arm with a staple gun, even after being able to lose his leg that can later be crudely stuck back on with first aid medication. These scenes were shocking in the moment and, while developers believed they could incorporate the same horrific moment with Grace, they thought it would take away if you had already seen it in Ethan. Rather than retread this horrific scene for Grace, which would be non-creative, they chose to focus their energy on new, horrifying experiences specific to Grace's narrative in Resident Evil Requiem.

New Scares Ahead For Players
While Grace may not experience the body mutilations suffered by Ethan, Resident Evil Requiem promises an abundance of frightful scares in the game. The player will still be able to choose to play in third-person or first-person, allowing them to experience it directly in Grace's eyes or in a slightly more distanced manner.

There is limited story exposition at this point in regards to Leon's involvement, or if Leon is in Resident Evil Requiem at all, but, if early write-ups are any indication, Resident Evil Requiem will build on the difference in story reasoning toward its strong return to survival horror, without having players be reintroduced to body horror scares again, or ultimately even the same scares as before.
